The World Health Organization (WHO) in 1948 defined health as a "state of complete physical, mental, and social well-being and not merely the absence of disease and infirmity."10 Although ideal for many people, this was an unrealistic goal. At the World Health Assembly in 1977, representatives of the member governments of WHO agreed that their goal was to have all citizens of the world reach a level of health by the year 2000 that allows them to live a socially and economically productive life.10 The U.S. Department of Health and Human Services in Healthy People 2010 described the determinants of health as an interaction between an individual's biology and behavior, physical and social environments, government policies and interventions, and access to quality health care.
